Rio Grande Hospital has launched a “community health needs assessment” survey to help it gain insights in the communities of Monte Vista, Del Norte, Center, Creede and South Fork.
Residents in those communities of Rio Grande County are being asked to participate in the survey, which can be found here.
“We are asking that you kindly take a few minutes and complete this survey so that we can better meet your future healthcare needs. The survey is anonymous and will only take you about 10 minutes to complete,” according to the survey’s introduction screen.
